Does this gift card expire?

There is no expiration however use is subject to card validation and medical evaluation for determination of eligibility.

Basic Terms and Conditions:

Usable up to the balance to purchase goods or services at MidAmerica Plastic Surgery. Card must be presented at time of redemption. This card is a “gift for you” and does not expire, nor does it charge any fees for any reason. Not a debit card or a credit card. No cash value. Limit 1 per customer per visit. Use of card for surgical prodecures subject to medical evaluation. MidAmerica Plastic Surgery is not responsible for lost or stolen cards. Purchase, use or acceptance of this card constitutes an acknowledgment of these terms and conditions.
