Does this gift card expire?

There is no expiration however use is subject to card validation and medical evaluation for determination of eligibility.

How to Purchase a Gift Card

We offer multiple purchasing options for your convenience - you can purchase via our online store, in-office, or by calling our office.

Basic Terms and Conditions:

Usable up to the balance to purchase goods or services at MidAmerica Plastic Surgery. The card must be presented at the time of redemption. This card is a “gift for you” and does not expire, nor does it charge any fees for any reason. Not a debit card or a credit card. No cash value. Use of card for surgical procedures subject to medical evaluation. MidAmerica Plastic Surgery is not responsible for lost or stolen cards. Purchase, use or acceptance of this card constitutes an acknowledgment of these terms and conditions. Gift cards may be purchased and redeemed for products and services with our Medical Spa at MidAmerica Plastic Surgery.
